If you are to do temporary work we will need you to complete the attached to give us permission to go to your previous Employer or Education for a reference. This is to allow them to release your details as part of data protection.

If you are going to be looking for temporary work, we shall require you to complete this form to give us your address history for the past 6 years.
Credit Check Consent Form
If we are required to complete a Credit Check on you then we will require you to complete this form. Note, we will not have to do this in all cases.
